For all incoming freshmen and newly enrolled students, please fill out the following form and return to your coach at cahsfootball@classicalacademy.com.

While the liability waiver covers your student beginning July 1st, in order to participate in summer workouts prior to July 1st we need to have the following form completed and on record.

Multi-Purpose Release and Medical Authorization Form for 2022-2023

Again, this form is for incoming freshmen and transfer students who did not attend CAHS last semester.  All returning CAHS students do not need to fill out this form.
